- One comprehensive examination, and one periodic examination per Year
- One Limited (emergency) dental examination per year, including one bitewing and two periapical X-rays
- Two preventive prophylaxis (“cleaning”) procedures OR two periodontal maintenance procedures per year
- Full-mouth X-rays
- Four bitewings and two periapical X-rays per visit
- Oral cancer screening
- 15% discount on all other dental services except for orthodontics (10 discount%) when paid for atthe time of service
